Senior Case Management
Under Senior Case Management, trained case managers and staff are
able assist vulnerable seniors and adults with disabilities in obtaining
community based support and service options.
Supportive Services include advocacy, assessment, consultations,
family support and informal counseling.
In-Home service options are available on a cost sharing basis for
individuals 60 years of age and older. Service options include personal
care, adult day health, home delivered meals, and minor home modification,
which also includes assistive technology, supplies and equipment.
