Oversight Nurse
The Human Services Council is seeking qualified applicants for the position of Oversight Nurse, The individual in this position will be responsible to receive referrals based on assessment of Medicaid Personal Care (MPC), Community Options Entry System (COPES) and/or Senior Personal Care (SPC) clients; review and assess client’s health status, personal care needs and current service plans; identify and coordinate medically related referrals and follow-up visits/reviews as needed in client’s home, adult family home or adult residential facility; review performance of client’s care provider; implement training or make training referrals; coordinate with medical professionals and provide information related to the health/medical condition of at-risk clients as necessary.

Case Aide \ Individual Providers
The Human Services Council is seeking qualified applicants to fill the position of Case Aide/Individual Providers. The person in this position will provide information, referral and assistance to clients including qualified seniors and adults with disabilities and their caregivers. It will be the primary responsibilities of this individual to manage Individual Provider contracts and duties related to the contracting process.

The Human Services Council is seeking qualified applicants for the position of Transportation Screener for the Brokerage Department. Duties include: screening, authorizing, and arranging transportation services for eligible clients including seniors and adults with disabilities to medical services and employment and training sites in Southwest Washington. Minimum requirements include a Bachelor's Degree and two years of customer service experience, additional relevant experience may substitute for up to two years of college. Degree verification and ability to pass a criminal background check is required.
